An investigation by the BC Cancer Agency into former Prince George oncologist Dr. Suresh Katakkar found four instances of "potentially preventable, treatment-related death."

The six-page document released Thursday indicates that 10 per cent of Katakkar's patients received "non-standard and unacceptable levels of care." Of those, eight cases were considered serious and resulted in preventable patient harm and four were considered critical.

"These are serious findings, and we have forwarded them on to the College of Physicians and Surgeons of British Columbia for further review," the report said.

The investigation found that Katakkar misdiagnosed some cases and failed to refer to other physicians; used incorrect protocols; failed to respond to warnings by colleagues and failed to follow medical standards.

Katakkar was suspended in May after the cancer agency launched the investigation and he resigned in June.
